Application (Membership Renewal) Instructions:

  • Provide all requested information in the application fields.

  • Fields with an asterisk (*) must be completed to submit the application.

  • Enter “NA” if “Not Applicable”.

  • Indicate a "Type of Membership" from the drop down box (e.g. Active Member, Student Affiliate, etc.)

  • When application is completed, click the “Submit” box – note that all fields must be completed for submission of your application.

  • Select method of payment and pay dues:

  • For payment by mail, include a personal note in your name indicating "HEC Annual Dues Payment" with a bank check or money order payable to Healthcare Executives’ Club, 4809 Avenue N, Suite # 260, Brooklyn, New York 11234.

  • For payment on-line by credit/debit card or PayPal, click the “PAY DUES” box (see below) and provide the requested information.
